Currently, nearly 6.5 million health plan members have access to the PHCS
Network. Health plans that use the PHCS Network are responsible
for providing their members with evidence that they are eligible to access the
PHCS provider network.
Most health plans will provide their members with identification cards that indicate an
affiliation with PHCS by including either a PHCS logo on the ID card, or by a PHCS phone
number on the ID card. In rare circumstances you will receive direct notification from
the member's health plan (via phone or fax) identifying PHCS as the member's network.

In some cases, initials or words inside the stripe under the PHCS logo are used to
describe in more detail Network services accessed by that member. The card may also list:
- Member information;
- Co-payment or encounter fee amount;
- Claims address; and
- Separate telephone numbers for obtaining eligibility and benefit information, for
Network referrals, if applicable, and for pre-certifying treatment.
We also recommend that you contact the patient's health plan to obtain eligibility
information before rendering any services. You can do this by calling the number on
your patient's identification card.

Please note that some members may continue to carry an ID card featuring the old PHCS
logo and corresponding horizontal stripe along the bottom edge.
Members carrying this older version of the ID card should receive the same appropriate
level of care as those carrying an ID card featuring our newer logo.
